General Practice significant event (SEA) and serious incident support
Somerset CCG supports the dissemination of learning and improvement in services via the General Practice Significant Event Audit Process and the reporting and investigation of Serious Incidents(SI).
The link below provides Primary Care with four easy-to-use tools that can improve how your practice uses SEAs as a learning tool:
All Significant Events should be reported by GP Practices via: email@example.com
The Somerset CCG Patient Safety team will then be in contact to discuss any support that may be needed.
Practices should use an approved tool for the Significant Event Audit Process. Click here to download the template.
When completed this template should be returned to the firstname.lastname@example.org mail box.
If a GP Practice is concerned that an event may be considered a Serious Incident (as defined in the NHSE Serious Incident Framework 2015 – see link below) please contact the Somerset CCG Patient Safety team for support and guidance on next steps.
